It's that time of year. Your employees with children who
are full-time college students must submit a completed
Student Coverage Questionnaire before December 31 to
ensure uninterrupted coverage into the 2005 calendar year.
You can find a copy of the questionnaire in your GBA
Handbook, and it is available to members under Forms and
Documents in the Member Services section of www.wellchoicenj.com.
Members must complete the form to include the school's
name and address, type of school and enrollment details,
and fax it to us at
1-800-780-1224 with either:
- Certification of full time status by the college registrar's
office - The employee will send the completed form with
a self-addressed stamped envelope to the registrar for
signature; OR
- A tuition bill showing full-time status in 2005, or school
letterhead showing full-time status, credits and enrollment
for 2005 signed by the registrar.
Please remind your employees of this policy to avoid
inconvenient interruptions in coverage. The employee is
responsible for submitting proof of eligibility as early as
possible before the start of the upcoming calendar year -
regardless of when documentation was submitted for the
current year - to ensure continued coverage as a
dependent full-time student.
Please refer to your group's benefit plan and
Schedule of Coverage to determine the extent and age to
which unmarried dependent children and dependent
full-time college students are covered.
